Practice Nurses Helen Jones, Elaine O’Connor, Katie Schofield, Fran Norris and Healthcare Assistant: Sharon Gibson We have a nurse available Monday to Friday 8.30am - 1pm and 2pm - 6pm. Appointments can be made for advice on minor accidents and illnesses, and infant immunisations. Doctors may also refer you to the nurse for advice and screening procedures. Our Community Midwives – Mandi Bradley & Jane Cotton – aim to offer continuity by providing care throughout pregnancy, birth and the initial postnatal period. Midwives Antenatal Clinics: Monday 1 pm – 3 pm Wednesday 1 pm – 3 pm Appointments with your doctor will be held during normal surgery hours. The midwives hold evening antenatal classes at the surgery. Health Visitors Anne Went & Chris Macklin. Tel No: 01252 873263 The Health Visitors role is to promote good health through education, support, advice and referral to other agencies. We work with individuals, families and the whole community. We can be contacted through the surgery or on the above number. Please contact Health Visitors for advice on minor childhood illnesses and immunisations. District Nurse Team Helen Constable, Olwen McNeil & Susan Curtis. Tel No: 01252 861461 The District Nurse Team provides skilled nursing care to patients who have an acute, chronic or terminal illness in their own homes and the GP surgery. District Nurses offer advice and support to patients, their families and carers to enable them to be as independent as possible. Counsellor Jackie Riley RGN. The services of a trained counsellor are available at the surgery. Referral will be made by your doctor. Youth Counselling (12-24 yrs) This is a free and confidential service available to young people. Please ring the surgery to make an appointment. Hartley Corner Patient Participation Group The main aim of this group is to improve communication and understanding between patients and doctors. The group may be able to contribute to improved services, and possible fundraising for the Patients Fund. This fund exists for the benefit of our patients. The fund purchases equipment and apparatus not available under the NHS. The group meets on an informal quarterly basis. Please contact the surgery for further information.
