Registration When you first join the Practice you may register with any of the doctors whose lists are open, and you will be invited to visit the Practice nurse for a registration check up. Home Visits If you require a home visit please request it before 11 am. Visits are usually performed between 12.30 pm – 2.30 pm. Please bear in mind that, on average, seeing you at home takes four times as long as seeing you in the Surgery. Emergencies etc. During surgery hours a receptionist will answer your call and contact the Duty Doctor by telephone. At night a telephone answering machine will give you the number to contact ‘Frimley Primary Care Services’ . For out of hours 6.30pm - 8.00am Monday to Friday and all weekend advice use the out of hours number Tel: 0118 936 5649 http://www.frimleypcs.org.uk/ This is a service run by the PCT. Repeat Prescriptions Please return the computer printout when you require prescriptions, with 2 working days’ notice. These can be collected from reception or posted on receipt of a stamped addressed envelope. Telephone requests are not accepted, - except for Age Concern patients. Test Results Please ring in the afternoon for results. We prefer specimens, such as urine samples, to be brought to the surgery before 3.30 pm in order to reach the laboratory that day. Confidentiality All patient records on file or in the computer are strictly confidential and can only be released to a third party (e.g. insurance company) by the patients written consent. All information is covered under the data protection act. Children over the age of 16 are treated as adults and their medical information and test results will not be divulged to their parents without their consent. Results of laboratory tests and x-rays will only be given to patients themselves or to the parents of minors if appropriate.
